Your Role and Responsibilities:
Conduct thorough assessments to understand patients' full clinical needs.
Review health insurance benefit plans to determine coverage options.
Collaborate with hospital partners, providers, and community services to coordinate care post-discharge.
Facilitate treatment plans, including discussions with patients and families to ensure the best care options.
Maintain accurate and detailed case records, including all interactions with internal and external teams.
Build strong, trusting relationships with medical teams and key stakeholders.
Advocate for patients by ensuring access to necessary resources and services.
Navigate clinically complex situations promptly, providing resolution within 1-2 business days.
Attend healthcare team meetings as needed to address patient care needs and remove barriers to care.
Consistently approach challenges with a problem-solving mindset and deliver solutions.
What We're Looking For:
Education: Bachelor's degree in Nursing or Social Work with an active and unencumbered Massachusetts license.
Experience: 3+ years of case management, managed care, or care coordination experience.
Skills: Strong communication, relationship-building, and problem-solving abilities.
Work Environment: Comfortable working remotely while staying engaged and collaborative with the team.
